Hi Chris,
The answer to most of your question can be found in VolcanoWorld itself,
in the "images of volcanoes" section. You could go to Cotopaxi
directly.
The summit elevation given is cheating since the base of Cotopaxi is
already at a high elevation. I don't think there is much of, if any, a
crater at the summit but I don't know for sure. Perhaps Dr. Chuck Wood,
founder of VolcanoWorld will weigh in with more details since he has
recently returned from Ecuador.
